- A counterfeited debit card is used to make unauthorized ATM withdrawals.
- ACH debits are posting to the account of a deceased Receiver listing an unknown Receiver name.
- RDFI takes an excessive amount of time to return an ACH debit, resulting in a potential loss for the ODFI.
- A business fails to make pay-or-return decisions in Positive Pay and later determines unauthorized debits were paid.
- An ODFI reviewing an Originator's oral authorization script identifies it fails to include revocation instructions.
